To enroll with Southwest Austin Caregivers, you must telephone our office in person. It is important that the prospective service
recipient knowingly request service. At this time, basic information, such as address, phone number, date of birth, etc., will be requested. |

If there is no one in the office when you call, please leave your name, and contact number with the answering service and a
volunteer will return your call at the earliest time possible. |

A volunteer will follow-up and make an appointment to visit with you in your home for a routine assessment. At that time you will be
provided with some helpful information and service guidelines will be explained. |

Upon completion of the initial in–home assessment, you will
become eligible to receive services from Southwest Austin Caregivers. |
